Customer Information / Application for Service Form
This form is required to set up new water service with Great Basin Water. Please complete all sections with your contact information, service address, and account details.
If you are a renter, it is very important that you also complete the Landlord Information section.
After submitting the form, if you have not heard from us within 48 hours, please call our office at 509-438-4450 to confirm your service setup.
Autopayment Form
This form allows you to set up automatic payments for your Great Basin Water account. By completing and submitting this form, your monthly bill will be paid automatically from your chosen card, ensuring your payments are always on time.
