Customer Information / Application for Service Form
This form is required to set up new water service with Great Basin Water. Please complete all sections with your contact information, service address, and account details.
If you are a renter, it is very important that you also complete the Landlord Information section.
After submitting the form, if you have not heard from us within 48 hours, please call our office at 509-438-4450 to confirm your service setup.
